Skilled Nursing program provides professional home medical care. Services are administered by clinically trained licensed professionals such as registered nurses (RN) and licensed practical nurses (LPN). This higher level of expertise helps clients who are managing ongoing complex medical conditions, and is available in shifts depending on patient needs.

Home Health Services Skilled Nursing Program may include but is not limited to:

  • Treatment and Injections

  • Supervisions of Medications

  • Dressing Change

  • Wound Care

  • Catheter Care

  • Restorative Care

  • Parenteral Nutrition

  • Diabetic Teaching

  • Care of Terminally Ill

  • Family/Patient Health Teachings

  • I.V. Fluids and Antibiotics Therapy

  • Pre-op and Post-op Care

  • Other Services not listed depending on staffing availability and patient needs

Intermittent and Shift Care provided but is not limited to:

  • Mechanical Ventilation, Bi-Pap, C-Pap

  • Tracheostomy Care and Changes

  • G-Tube Feeding/Changes, including NG- Tubes

  • Cather Care (Indwelling, Intermittent, Supra-pubic)

  • IV PICC Line- Including TPN/TPA/Blood draws

  • Chronic Wound Care – Amputations, Infections, Wound VAC

  • Diabetes, Hypertension, Cancer, Seizure Disorders

  • Respite Care, Medication Management, and Disease process training

  • Injections, Pain Management, Safety Evaluations

  • Pediatric care, well baby visits, Low birth weights

  • High Acuity Medically Fragile Patients
